For Current Student Organizations: Student Organization Yearly Registration Form
– This must be re-submitted each academic year in order for your organization to remain active and recognized.
Student Organization Annual Report Packet –
This report includes everything you need to close out your year, including a checklist, your new registration form, and your yearly financial report. Student Event Registration Form –
Submit this form to get clearance for any events or large-scale gatherings you might want to hold.
Waiver/Release of Liability – After requesting/receiving approval for student travel (submitted by sponsoring faculty/staff), these forms will be needed.
Release form should also be used for activities in which there’s a reasonable expectation of accident or harm (club sports, physical exercising, etc.). Solicitation and Sales Application –
If you decide to hold a fundraiser, you’ll need to fill out this form. Student Activity Request for Funding –
The Office of Student Engagement provides funding for student organization operations, travel for leadership and professional development seminars, and programs and activities. Social Media Account Request Form –
All official social media accounts representing Columbia State Community College and its departments and functions, must be established by the Columbia State Communications office, maintained by a designated account manager, and belongs solely to Columbia State Community College.
