Lewisburg Student Medical Claims

For any student who sought medical treatment due to the exposure on April 8, 2010, you may submit claims to the State of Tennessee and the City of Lewisburg. Information about the Tennessee Claims Commission can be found at  http://treasury.tn.gov/claims.    A “Claim for Damages” form can be downloaded from this link. A student wishing to file a claim with the state will need to use this form.  We are told this form will need to be completed by any student who had medical treatment due to the exposure and that it is recommended by the Claims Commission that you provide or attach a narrative of your claim to the form to best explain your circumstances. Columbia State and other Tennessee educational institutions have no control over this process and has no authority to bypass the state’s process, so any questions or issues regarding their process will need to be addressed to the Commission.
